Please take a moment and download the necessary forms for the Summer Arts Institute. These medical forms need to be filled out and signed by a doctor. These are due, at the latest, when you check-in. If you are having problems getting your doctor to sign these forms, please do not decide not to attend the Summer Arts Institute. If you call us, we will try to help.
