Medical insurance is provided for all full-time employees. The policy is through Blue Cross/Blue Shield of Georgia with a choice between two types of HMO: the HMO has only in-network benefits and the POS has an out-of-network benefit. |

Additional coverage for family members is available. Monthly rates can be obtained from the Office of Human Resources.
A Directory of Preferred Providers is available online from Blue Cross at www.bcbsga.com or call 1-800-441-2273 to check on any Provider you plan to see. The insurance goes into effect the date of employment and ends the date of termination.
Blue Cross administers our health insurance COBRA. Upon termination, Blue Cross will send employee and dependents COBRA information. |

A $10,000 ($5000 at age 70) life insurance policy is provided each full-time employee who is on our health insurance. An additional life insurance policy is provided which is equal to three times the annual salary, not to exceed $350,000. These benefits are provided at no cost to the employee. |

Accrues at 6 hrs per each full month worked prior to May 31, amount to be taken during first full fiscal year. Beginning second full fiscal year 80 hrs each year through fifth year. Eight hours is added beginning the 6th year and each year through 10th year for a maximum of 3 weeks or 120 hrs vacation after 10 years. Must be taken within the fiscal year. |

Accrues at 8 hrs per each full month worked to a maximum of 720 hrs. Of the sick leave accrued, 32 hours in each fiscal year may be taken as personal leave for such purposes as doctor, dentist, eye, etc., as needed. Personal leave does not carry over from year to year. |
